Just in my own personal experience regarding meds. I'm diabetic and was taking Trulicity, paying $25/mo for 4 weekly shots...one day the pharmacy hadn't adjusted my cost and it was listed full price and the clerk thought nothing of it when she said 'that will be $950'...umm, no, no it won't be. A few months ago I changed jobs and my new insurance company preferred a different shot (and oh joy, a daily not weekly one)...same cost, only I also get to pay for needles at another $15/mo...ok fine, but in changing medications I went thru weeks where I didn't understand why my vision was blurry...it was just the change in medication, only now clearing up.
Anyway, multiply our 2 'little' stories millions of times and you get what we've got.....a mess.
